WAIVER OF LIABILITY
I/We the undersigned, hereby give permission for the athlete to participate in AMAX Speed Training training program and events held by AMAX Speed Training. I further release Amax Speed Training from any responsibility of bodily injury to my athlete and understand that I am responsible for any medical fees accrued by my athlete as a result of medical attention. I certify that I am the parent/guardian of the athlete listed above and that all this information is correct. I respect the reserved right of AMAX Speed Training to suspend my athlete's activity if deemed necessary. I also understand that there are no refunds.
Social Media Waiver
I, the undersigned, do hereby consent and agree that Amax Speed Training, its Staff member, or affiliates have the right to take photographs, video, and or digital recordings of my athlete and use these in any and all media, Now or hereafter known, for any purpose by Amax Speed Training including but not limited to website use, editorial publications, catalog and advertising use.
